A beneficiary form is required to change a beneficiary. You can get the form one of two ways:
  1. We have an automated phone system that can send the form to you. To use the system, dial 1-800-523-0650. Select the option for "changes to a current contract", then select "beneficiary  change".

  2. Simply complete and submit the form below, and we will send you, via U.S. mail, the beneficiary form.
